Problem Description

Cisco is aware of two reported instances in which a power cord set manufactured by a contract supplier was wired improperly. Improper wiring may present a potential safety hazard, as further described in this Field Notice.

This potential defect is limited only to power cord sets that have the letters "LOR" on the label attached to the power cord sets. See Illustration 1 below.

This FN only applies to power cord sets made by one of Cisco's suppliers that were shipped with Cisco products. Linksys and Scientific Atlanta products were not shipped with the affected power cord sets, and therefore are not affected by the issue in this field notice.

Exceptions: Power Cord Sets That Are Not Affected

• Power cord sets specifically designed for China are not affected by the issue in this Field Notice.

• Power cords that are already operating with installed products are not affected.

• Power cord sets manufactured with a date code of "08 06" (August 2006) or later are not affected. (See Illustration 2 below).

• Power cord sets with a date code prior to "08 06"(August 2006) with a black dot on the label are not affected. (See Illustration 3 below).

• Power cord sets that do not have the letters "LOR" on the label are not affected.

Products Affected

The following power cord sets (excluding any power cord sets that fall within the exceptions listed above in the Problem Description section), are potentially affected:

Background

In the two known instances of mis-wiring, a reversal of the "live" and "ground" conductors in the appliance plug of the power cord set was found. If a mis-wired power cord set is connected to the power source, the chassis can become "live". Any contact with the chassis after it is connected to the power source could result in an electric shock.

Problem Symptoms

Signs or symptoms of mis-wiring cannot be detected by visual inspection and the mis-wiring is only evident when the power cord set is connected to the power source. Once that connection is made, an electric shock may result. Therefore, customers should NOT try to test any suspect cords themselves.

Before connecting the power cord set to a power source, please use the information described above to determine if a power cord set falls within the range of this notice and if it does, contact Cisco Technical Services Center (TAC).

Workaround/Solution

There is no workaround for this problem. The solution is to replace any suspected power cord.

What to Do With the Affected Power Cord Set

Cisco recommends that you discard any affected power cord sets through appropriate means. Do not attempt to connect a suspect power cord set identified through the method outlined above to any product or to the power source.
