Problem Description
A Cisco3845 using WIC-1SHDSL-V2 on HWIC slot2 with IOS images 12.3(14)T and 12.4(1) could result in a reset to ROMMON.
This condition happens only when the line-mode on the WIC in slot 0/2/0 is configured for 4-wire.
This behavior is observed only on 3845 and not seen on any other platform.
Background
This was only detected in Cisco's Engineering Labs. There have been no failures seen in the field.
A CISCO3845 may reset to rommon if the following three, low probability, criteria occur:

A WIC-1SHDSL-V2 is installed in HWIC2 slot (3rd slot from the right)
-
A WIC-1SHDSL-V2 is configured in 4-wire mode and DSL trains up. 2-wire mode will not produce this failure.

A Cisco3845 is using SW release 12.3.14T or the soon to be released 12.4.1. The future, second maintenance release of 12.4 will not have this issue.
If this problem is encountered with the Cisco3845-MB with the VID of "V01", it is recommended to power off the box and move the card to HWIC0, HWIC1, or HWIC3.
Problem Symptoms
A CISCO3845 may reset to rommon if the following three, low probability, criteria occur:

A WIC-1SHDSL-V2 is configured in 4-wire mode and DSL trains up. 2-wire mode will not produce this failure.

A Cisco3845 is using SW release 12.3.14T or the soon to be released 12.4.1. The future, second maintenance release of 12.4 will not have this issue.
If the above three criteria are met, the following screen dumps may be displayed from the CISCO3845:

Note: HWIC ports are marked via silkscreen as HWIC0, HWIC1, HWIC2, and HWIC3. The HWIC2 is the 3rd slot from the right on the CISCO3845-MB.
Workaround/Solution
We have no reported TAC cases to date for a problem with a WIC-1SHDSL-V2 failing in HWIC2 on the CISCO3845-MB.
This issue was only found in the Engineering Lab.
If this problem occurs with the Cisco 3845 MB with the VID of "V01", it is recommended to move the card to HWIC0, HWIC1, or HWIC3.
